

FREE Advance Medical Directive service
for Eligible Persons
Services include:
Explanation, witnessing, and making of Advance Medical Directives by registered medical practitioner.
Note: The free advance medical directive service is funded by the SIE Fund. Places are limited and offered on a first-come, first-served basis. Submitting this form does not necessarily mean you will be able to receive the free advance medical directive service.
Eligible Persons include:
1. Elderly households
Domestic households in which all members are aged 65 or above, excluding foreign domestic helpers; may include singleton, older couples, or households with multiple elderly persons
2. Persons with chronic diseases and their carers
Have been receiving continuous medical treatment, consultation or medication for certain type(s) of diseases (e.g., hypertension, diabetes, or other chronic diseases) for at least 6 months; but excluding medical treatment or consultations for the purpose of preventing a certain type of diseases
3. Single parents
Widowed, divorced, separated, or never married, and living with a child aged under 18
4. Subdivided unit households
Residing in units formed by splitting a unit of quarters into two or more “internally connected” and “externally accessible” units
